According to a witness, Maurry Cook, 44, decided the river was high enough to go over the dam in his kayak.
But Cook was caught in the spillway’s whirlpool and drowned after his kayak flipped over, the report stated from three witnesses’ accounts.
The incident occurred about 6:30 p.m.
Cook was pulled out of the river about a quarter-mile downstream at approximately 7:11 p.m. by firefighters, and EMS stated he was already deceased, according to the report.
Cook officially was pronounced dead at 7:45 p.m. by Justice of the Peace Endercio Chapa.
Also responding to the fatal accident were Department of Public Safety troopers and Texas Parks and Wildlife.
Three Rivers Police officers Howard Bostwick Jr. and Emmanuel Ambros investigated the accident.
Funeral arrangements are being handled by Galloway & Sons Funeral Director Inc.
Visitation is Thursday, May 17, from 9 a.m. to noon at First United Methodist Church. Funeral services will follow.
